What is Guided Tissue Regeneration (GTR)?
GTR is a method used to repair periodontal defects so that a tooth, or set of teeth, has more support and stability. Periodontal disease (periodontitis) is a process where bacteria trapped under the gums leads to a chronic infection. This causes the hard and soft tissues that support the teeth to break down. In certain cases, this leads to gaps that form between the teeth and bone. These gaps, or bony defects, often need to be treated with a separate procedure known as a bone graft. This is where material is placed into the defect to promote new bone growth.
Reasons for Guided Bone and Tissue Regeneration
Guided bone and tissue regeneration is used for many reasons. These include:
- Restoring bone in sockets where there has been a tooth extraction
- Preserving a bone socket for future prosthetic devices or dental implants
- Replenishing bone loss after removal of cysts (cystectomy) or impacted teeth
- Repairing bone defects after a periodontal wound has been reopened, usually to remove infected tissue
What are the Steps in a Guided Tissue Regeneration (GTR) Procedure?
-
Cleaning
The area under the gums is cleaned out to remove all bacteria.
-
Membrane Placement
A membrane is placed between the bone and gum tissue to prevent the gum tissue from growing into the bone defect and to promote new bone growth.
-
Healing
The bone and supporting structures will begin to heal and rebuild. It is important to maintain a good daily oral hygiene routine and continue regular dental care visits.
Use of Biologics in Guided Tissue Regeneration
In addition to traditional methods, we also use biologics to enhance the regenerative process. Biologics are substances that can stimulate the body’s natural ability to regenerate tissue. These can include growth factors and other biological agents that promote the activity of stem cells in the affected area, leading to faster and more efficient tissue regeneration.
Benefits of Biologics
- Stem Cell Stimulation: Biologics can stimulate the body’s stem cells, enhancing the natural healing process and promoting the growth of new, healthy tissue.
- Minimized Surgery: Using biologics can reduce the need for extensive surgical procedures, making the treatment less invasive.
- Faster Recovery: Patients often experience quicker recovery times and improved outcomes with the use of biologics.
Recovery from Guided Tissue Regeneration (GTR)
Recovery from guided tissue regeneration (GTR) typically takes one to two weeks, and patients may experience swelling and discomfort for a few days. To reduce swelling, patients can:
- Apply a reusable ice bag or frozen vegetable bag wrapped in a towel to the surgical area for 20 minutes on and 20 minutes off during the first 24–48 hours.
- Keep their head elevated above their heart.
- Avoid sleeping or lying on the side of surgery.
- Apply moist heat two days after surgery.
